Parents,
We have an opportunity to earn additional money using selected UPC Codes.   Publix has a box top program and periodically offers bonus points in conjunction with a $100 Gift Card sweepstakes.  If we send in 6, 8, or 12 UPC codes from the attached list, Publix will exchange the UPC codes for  30, 50, or 100 Bonus Box Top points worth $3, $5, or $10.  As mentioned above, the sweepstakes is a $100 Publix Gift Card and the form below is your entry ticket.  Here is what we would like for you to do:

1. Fill and cut out the address entry form below.
2. Cut and send in all the qualifying UPC labels (NOT box tops) you have.
3. Have your child place in an envelope with a filled out address label, and the cut out UPC codes, in the box in band room.
4. Wait to see if you have won the Sweepstakes from Publix.

The deadline for turn in is September 11, this is in order for me to meet the Publix deadline.  I will collect the deposited envelopes.
